Boss was a title bestowed upon individuals who displayed extraordinary fighting capabilities and services to their country. Only two individuals were known to have held the title of "Boss." A similar title known as "Big Boss" also existed. According to then-President Lyndon B. Johnson, the meaning behind the title "Big Boss" was meant to symbolize that the person had demonstrated skills and capabilities that were above that of even The Boss.
- The Boss (originally known as The Joy): the mother of U.S. Special Forces, the founder and leader of the Cobra Unit, the biological mother of Ocelot, the co-creator of CQC, and the mentor/spiritual mother of Big Boss.
- Big Boss, originally known as Naked Snake, was awarded the title in 1964 shortly after Operation Snake Eater: the founder and commander of FOXHOUND, the private military contractor of Outer Heaven, which originated from the Militaires Sans Frontières, and Zanzibar Land, and the genetic father of Solid Snake, Liquid Snake, and Solidus Snake.
- Venom Snake took up the title in 1984 after waking up from a coma due to a complex method of posing as Big Boss' body double. Previously, he was a medic in the Militaires Sans Frontières. Afterwards, he became the commander of Diamond Dogs and the official leader of Outer Heaven.
