In the Metal Gear series, a codename or title is awarded to the player upon completion of each game, often based on certain gameplay statistics. Originally introduced in Metal Gear Solid, this feature was also included in the re-released version of the earlier games.

Metal Gear

The requirements are fairly simple in that it only requires a specific time to finish the game, which in itself is divided into how long it takes to defeat each boss.

A codename is also awarded for completion of the Boss Survival mode.

Metal Gear Solid 2: Sons of Liberty

There are several gameplay statistics that determine which codename is awarded to the player. Early versions of the game also awarded the player with a clear code that could be submitted to the game's official website, recording these statistics on a leader board for each geographical region.[1]

ALERT MODE
When playing Tanker-Plant, there are three scripted Alert modes.

MECH DESTRUCTION
Destroyed cameras, both gun cameras and surveillance cameras, UAVs and Metal Gears. In both Extreme modes, the player has to destroy 20 RAYs to complete that boss fight, so they are allowed to destroy 39 cameras and UAVs.

SHOTS FIRED
Game counts every shot the player takes with a gun that fires bullets (lethal or non-lethal) and missiles. Weapons that don't count are: Cooling Spray, Magazine, any kind of grenade, empty magazine and H.F. Blade.

CLEARING ESCAPE
Number of times the player has escaped Clearing Mode.

MISSION FAILED
The table below refer to the secret stats shown on Plant's Mission Failed screen. The player can die after saving to examine the current stats for the playthrough.

Peace Walker codenames.

Because of various gameplay changes in Peace Walker, codenames, unlike previous games, do not require the completion of a playthrough to gain it. Instead, it only requires beating various missions a number of times. Likewise, the requirements for gameplay have been reduced to the types of weapons used to defeat enemies, the player mode it was done in, and whether the use was lethal or non-lethal for the most part. In addition, insignias are issued for the various tasks involving the (lack of) use of recovery items, (not) triggering an alert, among others.

Metal Gear Rising: Revengeance

Although not Codenames, a similar ranking system is utilized in Metal Gear Rising: Revengeance, which can be accomplished by completing certain objectives.

Title Objectives
Thunder God Obtain an overall S-rank for all chapters, on all difficulty levels.
Lightning Bolt Obtained an overall B-Rank or higher on all difficulty levels.
Divine Wind Complete the game on Revengeance difficulty within three hours without dying.
Cyclone Complete the game on Very Hard difficulty within three hours without dying once.
Gale Force Complete the game on Hard difficulty within four hours without dying once.
Strong Wind Complete the game on Normal difficulty within four hours without dying once.
Light Breeze Complete the game on Easy difficulty within five hours without dying once.
Thunderstorm Complete the game on Hard difficulty or above without dying, and without using a single unit of Repair Nanopaste.
Passing Rain Complete the game on Normal difficulty or higher in less than eight hours.
Rain Shower Complete the game in 10 hours or more, with 10 deaths or more, and use in excess of 20 Life recovery systems or more.
Lightning Speed Complete the game on any difficulty within two hours.
Light Drizzle Complete the game on Normal difficulty or higher in 12 hours or more.
Mr. One Percent Earn at least 300,000 BP during a playthough on Normal difficulty or higher.
Massive Deterrent Complete the game on Normal difficulty or higher without killing a single Cyborg, Armored Cyborg, or Cyborg Berserker.
Samurai Complete Normal or higher difficulty while using the High Frequencey Blade as a weapon.
Battle Tested Complete Normal or higher difficulty despite being discovered by enemies 50 or more times.
Jack the Ripper Cut your enemies at least 1000 times using Blade Mode.
